Types of Office Visits Well Child Visit Sick Visits Walk Ins Family Planning/STD Check Ups   No Appointment Needed Hospital Visits   Well Child Visit These are visits that usually follow the CHDP schedule. Please plan to spend atleast 45 minutes in the clinic for this visit. There are certain requirements each well child visit. Such as Measurements: - heights, weight, or head circumference, labs, hearing and vision screening. Immunization: - unless they have a fever or a serious infection, all children are to be updated on their immunization when they come for a well child visit. Nutritional Assessments: - all children are to fill out a nutrition assessment form when they come for a well child visit. Health Education Behavioral assessments (HEBA): - these forms are to be filled out once per year. They are to be completed by the parent or by the patient if the patient is 11 years or older.
