Primary purpose for existence of this clinic is:
To provide QUALITY health care to a medically underserved population.
Each individual is to be treated with respect, courtesy, and dignity.
All patient information is handled in a confidential amnner.
- All complaints/compliments are to be directed to the office manager.
- Children under 18 years are to be accompanied by their parent or legal guardian.
Only those who are seeking contraceptives, STD testing/treatment, or pregnancy testing/counseling
are allowed to come on their own. Occasionally, a patient may send an older sibling in with
another child for an examination. This is permissible only if the parent provides a signed
written permission note.
- Requests for medical information, medical records, and shot records from other
offices must be accompanied by a consent for release of medical records signed by the
patient, or the legal guardian. We cannot release any medical records without this consent.
- Patients will occasionally come in or call for a copy of their medical records
and/or immunization records. These require at least 24 hours for preparation. The requested
information will be pulled for review by the physian who will notify the appropriate staff
when it is ready for copying. Immunization records are not to be copied on the copying machine.
Instead, a new card may be issued and the date of issue is documented in the patient file. If a
second card is requested a $5.00 fee will be applied.
- Patients who have forms that require a physician's signature are to be notified
that there is a 24-hour turn around time. This especially holds true for physician forms, forms
for verification of immunization status and letters for the school Each of these requests costs
$15.00.
